Resources & Services for Adults

The Adult Survivorship Program is open to all adults (age 21 or older) who have been diagnosed with cancer, regardless of where treatment was received or type of cancer.  Cancer survivors may start to participate in the FREE health and wellness activities at any time after they have received the diagnosis of cancer.

Upon completion of treatment, visit with our Oncology Certified Registered Nurse to receive:

  • Adult Treatment Summary – a comprehensive review of your cancer treatment from diagnosis to completion
  • Survivorship Care Plan – a summary of potential late effects, symptoms and treatment, recommendations for cancer screening; psychosocial effects, financial issues, recommendations for a healthy lifestyle, effective prevention options, and a list of support resource
